Upon receipt of the information posted on this form the LHSWCA secretary will create a receipt/invoice for the coaches listed in the form.

The receipt/invoice will be mailed to a provided address or faxed to a provided number.

Upon receipt of this information the members requested will be listed on the Members" and "Financial" pages of the LHSWCA Website.  However, voting privileges will not be granted until dues are received.
